This survey had been part of the website https://future-democracy.org for almost 10 months.
It was shown only after the book PDF was downloaded.
Obviously though, almost nobody actually answered the survey after downloading the PDF.

Therefore, I removed the survey from the website for brevity there, and start it again here.
